How to access funding for our Services

Arbor Therapy has officially partnered with Parenting Special Kids Network (PSKN), an Arizona nonprofit dedicated to supporting families of children with disabilities. PSKN provides FREE, one-on-one support to families by creating individualized plans based on their most urgent needs. Their services include:

Assisting with Arizona Services such as DDD, AHCCCS, ESA, AzEIP & ALTCS applications Assisting with Federal Services such as SSI, DAC, SSDI & Survivor Benefits program
Helping secure long-term housing, employment, and transportation
Supporting families with POA, trusts, and future planning
Connecting families to expert resources (including attorneys and former DDD coordinators)
Parent to Parent Support Groups
One to One Assistance
and many more!

How to apply for services with Division of Developmental Disabilities (DDD), for Phoenix and throughout the state of Arizona

*DDD can accept your application packet in one of three ways:
*You can email the completed application and all required documentation to DDDApply@azdes.gov. (This is the fastest way to process your application.)
*You can mail the completed application and all required documentation to any DDD office statewide.
