Arbor Therapy

PSKN Partnership

  • Arbor Therapy has officially partnered with Parenting Special Kids Network (PSKN), an Arizona nonprofit dedicated to supporting families of children with disabilities. PSKN provides FREE, one-on-one support to families by creating individualized plans based on their most urgent needs. Their services include:
  • Assist with Arizona Services such as DDD, AHCCCS, ESA, AzEIP & ALTCS applications
  • Assist with Federal Services such as the SSI, DAC, SSDI & Survivor Benefits programs
  • Help secure long-term housing, employment, and transportation
  • Support families with POA, trusts, and future planning
  • Connect families to expert resources (including attorneys and former DDD coordinators)
  • Parent to Parent Support Groups
  • One to One Assistance
  • Additional family-centered resources and individualized support based on each family’s needs

How to Apply for Arizona Long Term Care System (ALTCS)

To apply, you need to complete an application.

  • To apply, you will need to complete an application.
  • To get started, call ALTCS toll-free at 888-621-6880.
  • A family member or trusted individual may assist you or apply on your behalf.
  • You will be asked to provide documentation to verify financial and non-financial eligibility.
  • Applicants must be determined to require a nursing home level of care.
